What city do you reside in? Your city decides which assignment you are considered for: Las Vegas is the ongoing role, and Phoenix, Salt Lake City and New York are one-off cards on a single date. If yours is not listed, choose "Other" — we keep applications on file for cities the UFC adds later.

How far are you willing to travel to the venue for assignments? (max) Travel is fully covered, including rideshare and transit.

Are you available for cards throughout the year? Cards last five to six hours. Answer for the assignment you are applying to: Las Vegas is ongoing, with a card most weeks — Contender Series on Tuesday nights, Fight Nights on Saturday afternoons — so Tuesdays matter there. A one-off road card in Phoenix, Salt Lake City or New York is a single Saturday, and needs no other availability; "Weekends only" is a full yes for those.

Briefly describe your MMA background — any combat sports you've trained or competed in, and for how long. Include how closely you follow the UFC, and roughly how many cards you watch a year.
